腰骶移行椎Castellvi和陈崇文等分型可信度与可重复性比较及意义

Confdence and repeatability of Castellvi classifcation system and novel classifcation system for lumbosacral transitional vertebrae

  • 摘要: 目的 比较腰骶移行椎Castellvi分型和陈崇文等分型系统的可信度和可重复性,探讨陈崇文等分型的临床价值。 方法 选取住院及门诊治疗资料较完整患者80例,其中男47例,女33例。年龄16 ~ 55岁,平均36.7岁。每例均有完整的腰椎X线正侧位片、移行椎三维CT重建及腰骶MRI片。由4名有分型经验的脊柱外科医师分别进行Castellvi分型和陈崇文等分型,2周后再次进行分型,收集结果后对分型可信度和可重复性进行分析。计算Kappa检验的一致性。 结果 Castellvi分型和陈崇文等分型可信度分别为84.83%(Kappa=0.789)、87.67%(Kappa=0.892),可重复性分别为90.75%(Kappa=0.895)、93%(Kappa=0.914)。 结论 陈崇文等分型直观、简洁、易记,对治疗有指导意义。

     

    Abstract: Objective To compare the confidence and repeatability of Castellvi classification system and novel classification system for lumbosacral transitional vertebrae and study the clinical value of novel classifcation system. Methods Eighty patients (47 males and 33 females) aged 16 -55 years (mean 36.7 years) were included in this study. Arterioposterior and lateral lumbar vertebrae X-ray flm, transitional vertebrae 3D CT reconstruction flm, and lumbosacral vertebrae MRI flm were taken for each patient. The flms were classifed by 4 experienced spine surgeons according to the two classifcation systems and repeated after two weeks. The confdence and repeatability of the two classifcation systems were compared. The consistency of Kappa test was calculated. Results The confdence of the two classifcation systems was 84.83% (Kappa coeffcient=0.789) and 87.67% (Kappa coeffcient=0.892), respectively. The repeatability of the two classifcation systems was 90.75% (Kappa coeffcient=0.895) and 93% (Kappa coeffcient=0.914), respectively. Conclusion The novel classifcation system is more objective, succinct, and easier to keep it in heart than the Castellvi classifcation system, and can thus be used in treatment of vertebral diseases.
