超声三平面法测量心肌梗死患者左心室整体收缩功能的临床应用价值

Clinical application value of Tri-plane in measuring left ventricular systolic function in patients with myocardial infarction

  • 摘要: 目的 探讨超声三平面法测量心肌梗死患者左心室整体收缩功能的临床应用价值。 方法 选取2013年1 - 12月在我院心内科住院的心肌梗死患者59例(前壁梗死29例,后壁梗死18例,侧壁梗死12例),均在心肌梗死发生后1个月内应用三平面法及Simpson's法分别测得左心室射血分数(left ventricular ejection fraction,LVEF),比较两组间参数,并分析两者的相关性。可重复性研究:随机抽取20例受检患者,由同一观察者不同时间及两位观察者分别用三平面法测量每位受检者的左心室射血分数,分析同一观察者及观察者间的相关系数。 结果 59例中有5例因内膜面显示不清,无法用Simpson's法测量左心室整体收缩功能,其余患者三平面法与Simpson's法测量的LVEF结果相关性良好,相关系数为r=0.92。同一观察者不同时间及两观察者间用三平面法测量20位受检者的左心室射血分数,相关系数分别为0.83P< 0.01)和0.81P< 0.01)。 结论 三平面法测量左心室射血分数,操作简便,且与Simpson's法相关性良好,重复性较好,有一定的临床应用价值。

     

    Abstract: Objective To discuss the clinical application value of Tri-plane in measuring left ventricular systolic function in patients with myocardial infarction. Methods Fifty-nine patients with myocardial infarction in our hospital from January to December in 2013 (29 cases with anterial segment infarction, 18 cases with posterial segment infarction, 12 cases with lateral segment infarction) were enrolled in this study, their left ventricular ejection fraction (LVEF) were measured using Tri-plane and Simpson's within one month, the parameters were compared between the two groups and the correlation between the two methods were analyzed. Repeatability study: Twenty patients were randomly selected, their LVEF were detected using Tri-plane by the same observer after two days and then two observers, the correlation between the two methods were analyzed. Results Of the 59 patients, LVEF of 5 cases could not be measured due to the unclear membrane surface display, the correlation of other patients’ LVEF detected by Tri-plane and Simpson's was good with the correlation coefficient of r=0.92. The correlation coefficient of the two methods was 0.83 (P< 0.01) and 0.81 (P< 0.01), respectively. Conclusion Measuring LVEF with Tri-plane is a simple and accurate method, and it shows good correlation with Simpson's method with good repeatability, which is of great clinical application value.
