超声诊断视盘凹陷在评价重症化学性眼外伤手术预后中的价值

Evaluation of preoperative ultrasonography with optic disk depression in patients with severe chemical injury

  • 摘要: 目的 评价术前超声诊断视盘凹陷在重症化学伤患者复明手术预后中的临床价值。 方法 回顾性分析2010年1月-2013年6月在本院因重症化学性眼外伤晚期欲行BostonⅠ型人工角膜移植术复明的住院患者共28例(28眼)病例资料,所有患者术前进行闪光视觉诱发电位检查和常规B型超声观察视盘凹陷。根据超声下视盘处球壁形态超声诊断结果分为有/无视盘凹陷。所有病例均一期完成Boston Ⅰ型人工角膜移植术。术后两周检查最佳矫正视力,分析超声诊断视盘凹陷在重症化学伤患者复明手术预后中的价值。 结果 28例中,术前超声诊断有视盘凹陷5例,术后最佳矫正视力(1例0.05,1例0.1,1例0.12,2例指数/眼前)。术前超声诊断无视盘凹陷23例,术后最佳矫正视力范围为0.1 ~ 0.8(0.5±0.36)。 结论 术前超声诊断视盘凹陷可以准确判断重症化学性眼外伤患者复明手术预后视力较差。

     

    Abstract: Objective To assess the evaluation of preoperative ultrasonography with optic disk depression in patients with severe chemical injury. Methods Clinical data about 28 patients (28 eyes) with chemical burns in the eyes with severe tissue damage and prepared to undergo the arti ficial cornea transplant (Boston Ⅰ) in our hospital from January 2010 to June 2013 were retrospectively analyzed. The preoperative examination was performed in 28 eyes using Flash Visual Evoked Potentia (F-VEP) and B-scan ultrasonography, respectively. The results were divided into optic disc depression and non optic disc depression according to the eyeball wall morphology, and all patients had undergone the artificial cornea transplant (Boston Ⅰ). The images and data were analyzed and compared with the operation results. Results In 28 cases, there were 5 cases of optic disc depression, and the best corrected visual acuity of postoperative were 0.05 in 1 case, 0.1 in 1 case, 0.12 in 1 case, finger count/1 m in 2 cases. Preoperative ultrasonic diagnosis without optic disc occurred in 23 cases, the best corrected visual acuity of postoperative ranged from 0.1- 0.8 (0.5±0.36). Conclusion The B-scan ultrasonography can observe the prognostic visual accurately by optic disk depression in patients with chemical burns eyes.
