慢性炎性脱鞘性多发性神经病临床及电生理特点

Clinical and electrophysiological characteristics of chronic inflammatory demyelinating polyneuropathy

  • 摘要: 目的 探讨慢性炎性脱髓鞘性神经病(chronic inflammatory demyelinating polyneuropathy,CIDP)的临床及电生理特点。 方法 对2001 - 2011年确诊的56例CIDP患者的临床特点进行分析,并对所有患者进行神经传导速度、波幅、潜伏期及肌电图测定,与32例正常组肌电图进行对照。 结果 患病组脑脊液检查47例(83.9%)表现为蛋白-细胞分离现象,所有患者肌电图均表现神经源性损害,两组间运动传导除近端潜伏期比较无统计学差异,其他各项差异均有统计学意义。 结论 CIDP存在广泛的周围神经损害,存在以脱髓鞘为主伴轴索变性的电生理改变。

     

    Abstract: Objective To investigate the clinical manifestation, electrophysiologic features of patients with chronic inflammatory demyelinating polyneuropathy (CIDP). Methods Fifty-six patients diagnosed as CIDP were enrolled in this study, and their clinical and electrophysiologic features were analyzed. All patients had undertaken the electrophysiological studies and their nerve conductive velocity, amplitude, latency and electromyography were tested and analyzed. The value of those patients was compared with that of the 32 normal persons. Results Protein - cell separation phenomenon were performed in 47 cases (83.9%), and the EMG of patients were showed neurogenic damage. Motor conduction in addition to the proximal incubation period between the two groups had no significant difference, while other comparisons between the two groups had statistical significance. Conclusion CIDP shows widespread damages, and there exists electrophysiological changes given priority to demyelinating with axonal degeneration.
