冠心病患者血清γ谷氨酰转移酶水平与颈动脉弹性力学改变的相关性

Relationship between serum gamma glutamyltransferase level and carotid artery elasticity in patients with coronary heart disease

  • 摘要: 目的 分析血清γ谷氨酰转移酶(gamma-glutamyltransferase,GGT)水平与颈动脉血管弹性力学改变的相关性。 方法 选取2013年1月-2014年5月就诊于解放军总医院心内科冠心病患者92例,其中男54例,女38例。根据颈动脉斑块是否形成分为斑块组和非斑块组,另选取无冠心病正常体检者40例为对照组。分别检测各组受试者血清GGT水平及血脂、血糖等生化指标,对所有受试者行颈动脉速度向量成像(velocity vetor imaging,VVI)检查,测量并记录颈动脉各节段最大径向运动速度(Vs)、最大切向应变(Smax)、最大切向应变率(SRmax),取各节段平均值,比较3组参数的差异,并分析血清GGT水平与颈动脉弹性力学变化的相关性。 结果 3组受试者血清GGT水平两两比较均有差异(P< 0.05)。斑块组各节段平均Vs、Smax、SRmax低于非斑块组、对照组(P< 0.05),非斑块组各节段平均Vs、Smax、SRmax低于对照组(P< 0.05)。冠心病血清GGT水平与颈动脉各节段平均Vs、Smax呈负相关,(rVs=-0.683、rSmax=-0.478,P< 0.01)。 结论 冠心病患者血清GGT水平与颈动脉血管弹性力学变化呈负相关。血清GGT水平联合颈动脉VVI检测对心血管疾病早期诊断及病变程度评估具有指导意义。

     

    Abstract: Objective To analyze the correlation between serum gamma glutamyltransferase level and carotid artery elasticity. Methods A total of 92 cases (54 males, 38 females) with coronary heart disease (CHD) admitted to department of cardiology in Chinese PLA General Hospital from January 2013 to May 2014 were enrolled. All patients were divided into CAP and non-CAP group according to carotid artery plaque, and 40 subjects of non-CHD were severed as controls. The serum levels of GGT, TC, TG, GLU were determined in all subjects. The images of common carotid stored in short axis view were obtained and analyzed by VVI technique. The maximum radial velocity (Vs), maximum tangential strain (Smax), maximum tangential strain rate (SRmax) of each segment of carotid were measured and recorded, and the averages of all segments were taken. Differences of three groups were compared and analyzed. The correlation between serum GGT levels and the elasticity changes in carotid were explored. Results Differences of serum GGT levels were found in three groups (P< 0.05). The segments average Vs, Smax, SRmaxof CAP group were lower than those of non-cap and control group (P< 0.05), and they were lower in non-CAP group than controls (P< 0.05). The serum GGT levels in patients with CHD were correlated with average Vs, Smaxnegatively, with the correlation coefficient of-0.627,-0.478 (P< 0.01). Conclusion The negative correlation between serum GGT level and elasticity mechanical changes of carotid artery is found in patients with CHD. Serum GGT level combined with carotid VVI detection has guiding significance in early diagnosis and severity assessment of cardiovascular disease.
