经电子支气管镜氩等离子体凝固术治疗大气道阻塞72例临床分析

Argon plasma coagulation in treatment of patients with central airway obstruction under bronchoscope: An analysis of 72 cases

  • 摘要: 目的 评价经电子支气管镜氩等离子体凝固术(argon plasma coagulation,APC)治疗大气道阻塞的有效性及安全性。 方法 回顾性分析本院呼吸内科2004年9月1日-2014年9月1日行APC治疗患者的临床资料,观察疗效及不良反应发生情况。 结果 72例大气道阻塞患者,男58例,女14例,平均年龄(55.0±14.5)岁,共行205次APC术,术中及术后均未出现危及生命的严重并发症。72例治疗后气促指数改善(u=6.972,P< 0.01)。KPS评分由治疗前(74.8±11.7)分升至(84.5±18.4)分(t=7.61,P< 0.01)。疗效:完全有效20例(27.78%),部分有效47例(65.28%),共计有效67例(93.06%),无效5例(6.94%)。 结论 经电子支气管镜氩等离子体凝固术治疗大气道阻塞,疗效满意,安全性高。

     

    Abstract: Objective To evaluate the efficacy and safety of argon plasma coagulation therapy for central airway obstruction under bronchoscope. Methods Clinical data about patients who underwent argon plasma coagulation (APC) therapy in department of respiratory of Chinese PLA General Hospital from September 2004 to September 2014 were retrospectively analyzed. Their clinical efficacy and side effects were observed. Results Seventy-two patients (male 58, female 14) with an average age of (55.0±14.5)years had received APC therapy for 205 times. No major complications were found during or after operation. The postoperative dyspnea index was lower than the preoperative ones (u=6.972, P< 0.01). KPS increased from 74.8±11.7 to 84.5±18.4 significantly(t=7.61, P< 0.01). Of all patients, 20 (27.78%) were successfully treated, 47 (65.28%) were partially successfully treated, and 5(6.94%) were treated invalid. Conclusion APC is an effective and safe method for central airway obstruction resection.
