膝关节置换术中内侧副韧带浅层贴骨剥离松解的效用分析

Effects of shallow attached bone stripping release of medial collateral ligament in total knee arthroplasty surgery

  • 摘要: 目的 分析膝关节置换术中内侧副韧带浅层贴骨剥离技术在软组织平衡中的效用。 方法 选取解放军总医院2013年6月1日- 2014年8月15日膝关节置换病例44例,按照是否采用内侧副韧带浅层贴骨剥离技术分为对照组与实验组,比较实验组松解前后内侧胫股间隙的变化情况以及两组术后活动度和术后3个月膝关节功能评分。 结果 内侧副韧带浅层贴骨剥离技术可以分别为屈曲胫骨间隙和伸直胫股间隙提供(1.88±1.05) mm及(2.23±1.00) mm的松解度;两组术后活动度及术后3个月膝关节功能评分差异无统计学意义。 结论 内侧副韧带浅层贴骨剥离带来的松解度有限,若术中发现截骨后膝关节内侧胫股间隙过于紧张,则需要结合其他技术综合平衡内外侧间隙。

     

    Abstract: Objective To analyze the effects of shallow attached bone stripping release of medical collateral ligament in the soft tissue balancing. Methods Forty-four cases with knee prosthesis admitted to Chinese PLA General Hospital from June 1 , 2013 to August 15, 2014 were selected, patients were divided into control group and experimental group according to whether shallow attached bone stripping release of medial collateral ligament was adopted or not. The change status of medial tibia clearance of experimental group before and after release, the activity of both groups after operation, and the difference of knee function grading 3 months after operation were compared. Results The shallow attached bone stripping release of medial collateral ligament could provide (1.88±1.05) mm and (2.23±1.00) mm releasing degree in the buckling tibia clearance and straight shank stock clearance. The difference of activity degree and knee function score at 3 months after operation was insignificant. Conclusion The releasing caused by the attached bone stripping release of the superficial medial collateral ligament is limited. If the medial gap of the knee joint after osteotomy during the operation is too tense, it requires other methods to balance the lateral and internal gaps.
