异种脱细胞真皮基质在口腔颌面部创面修复中的应用

Application of heterogeneous acellular dermal matrix in repairing oral and maxillofacial defect

  • 摘要: 目的 评价异种脱细胞真皮基质修复膜用于口腔颌面部创面修复中的临床效果。 方法 收集2011-2014年解放军总医院海南分院口腔科59例因肿瘤、外伤、黏膜病变、瘢痕切除术等原因引起的口腔颌面部缺损,应用异种脱细胞真皮基质修复膜进行修复,缺损部位为颊、腭、舌、口底、腮腺、牙龈、前庭沟等,术后随访2周~ 6个月,并进行术后追踪随访及修复效果评估。 结果 共修复口腔颌面部各类创面59例,一期愈合52例,成功率达88.14%。7例因创面较大,且受植区创面不平整、不规则,与修复膜之间存在死腔,以及过早的张闭口运动等因素,导致修复膜与创面部分脱落。 结论 异种脱细胞真皮基质在口腔颌面部各类创面修复中起到了创面覆盖、引导组织再生和支架作用,修复效果满意,值得临床推广。

     

    Abstract: Objective To evaluate the clinical effect of heterogeneous acellular dermal matrix (H-ADM) in oral and maxillofacial defect repair. Methods Fifty-nine cases with oral and maxillofacial defects which were caused by tumor surgery, trauma, mucosal disease and scar resection were restored with H-ADM. The defects were located in buccal region, palate, tongue, foor mouth, parotid gland, gingiva, oral vestibular groove and so on. All patients were followed up for 2 weeks to 6 months postoperatively and the repair effcacy was assessed. Results Of the 59 cases undergoing transplantation, 52 cases were primary healing with the success rate of 88.14%, 7 cases failed due to large, tough and irregular wound, cavity existing between repair membrane and wound, early mouth movement and so on, which caused the falling off of partial repaired membrane. Conclusion H-ADM plays a role in wound coverage, guide tissue regeneration and biological scaffold during wound healing. The effect of repairing is satisfactory and it is worthy of clinical promotion.
