结直肠癌组织Endoglin标记的微血管密度与淋巴结及远处转移相关性的Meta分析

Meta analysis of correlation between microvessel density for endoglin and lymph node metastasis and distant metastasis in colorectal cancer

  • 摘要: 目的 分析结直肠癌患者癌组织中Endoglin(CD105)标记的微血管密度(microvessel density,MVD)与淋巴结及远处转移的相关性。 方法 利用万方数据库、清华同方电子数据库(CNKI)、维普中文期刊数据库,以“结直肠癌、大肠癌、结肠癌、直肠癌、endoglin、CD105”为关键词,检索2000年1月-2014年10月与结直肠癌组织中Endoglin标记的MVD与淋巴结转移及远处转移关系相关的研究文献,并对文献进行筛选和评价,符合标准的文献用Revman5.3软件进行Meta分析。 结果 Endoglin标记的MVD与淋巴结转移相关文献入选12篇,共741例患者,有淋巴结转移373例,无淋巴结转移368例,异质性检验提示各研究结果间存在异质性(χ2=136.86,P< 0.01,I2=92%),采用随机效应模型进行合并,有淋巴结转移的结直肠癌组织和无淋巴结转移的结直肠癌组织比较,Endoglin标记的MVD的加权均数差(weighted mean difference,WMD)为9.89,95% CI为(6.92,12.85),提示有淋巴结转移的结直肠癌组织中Endoglin标记的MVD明显高于无淋巴结转移的结直肠癌组织(Z=6.54,P< 0.05);Endoglin表达与远处转移相关文献入选6篇,共305例患者,远处转移79例,无远处转移226例,异质性检验提示各研究结果间存在异质性(χ2=77.28,P< 0.01,I2=94%),采用随机效应模型进行合并,有远处转移的结直肠癌组织和无远处转移的结直肠癌组织比较,Endoglin标记的MVD的WMD为7.9,95% CI(1.91,13.88),提示有远处转移的结直肠癌组织中Endoglin标记的MVD明显高于无远处转移的结直肠癌组织(Z=2.59,P< 0.05)。 结论 在结直肠癌患者中,发生淋巴结转移和远处转移的患者癌组织中Endoglin标记的MVD明显升高,Endoglin的检测对筛选高危转移者具有重要意义。

     

    Abstract: Objective To investigate correlation between microvessel density (MVD) in endoglin (CD105) and lymph node metastasis, distant metastasis in the specimens of colorectal cancer. Methods Systematic searches of Wanfang, CNKI and cqvip databases meeting specifed search criteria were performed from January 2000 to October 2014 with “colorectal cancer, colon cancer, rectal cancer, endoglin, CD105” as key words, and the data meeting the selected standard were collected, then Meta-analysis was carried out by Revman 5.3. Results Twelve researches about the correlation of MVD for endoglin and lymph node metastasis were selected based on the standard. There were 741 patients, including 373 patients with lymph node metastasis and 368 patients without lymph node metastasis. Heterogeneity was found in these reports (χ2=136.86, P< 0.01, I2=92%). Random effects model was applied, and colorectal cancer tissues with or without lymph node metastasis were compared. The weighted mean difference (WMD) of MVD for endoglin was 9.89 (95% CI= 6.92 -12.85). The MVD for endoglin of colorectal cancer tissue with lymph node metastasis was signifcantly higher than that of colorectal cancer tissue without lymph node metastasis (Z=6.54, P< 0.05). Six reports about the correlation of MVD for endoglin and distant metastasis were selected based on the standard. There were 305 patients, including 79 patients with distant metastasis and 226 patients without distant metastasis. Heterogeneity was found in these researches(χ2=77.28, P< 0.01, I2=94%). Random effects model was applied, and colorectal cancer tissues with or without distant metastasis were compared. The WMD of MVD for endoglin was 7.9 (95% CI= 1.91-13.88). The MVD for endoglin of colorectal cancer tissue with distant metastasis was signifcantly higher than that of colorectal cancer tissue without distant metastasis (Z=2.59, P< 0.05). Conclusion In colorectal cancer patients, higher MVD for endoglin is observed in cancer tissues with lymph node metastasis and distant metastasis in colorectal cancer patients. The detection of MVD for endoglin in colorectal cancer has great signifcance in screening patients with high risk of metastasis.
