乳腺浸润性导管癌X线征象与Ki-67、p53表达的关系及临床意义

Relationship between mammographic appearances of breast infiltratingductal carcinoma and expression of Ki-67 and p53 and its clinical significance

  • 摘要: 目的 探讨乳腺浸润性导管癌X线征象与Ki-67、p53表达的关系。 方法 回顾性分析2014年8月- 2015年9月辽宁省医学院附属第一医院和辽宁省抚顺市第四医院放射科经术后病理证实为乳腺浸润性导管癌189例的X线征象,并分析其与Ki-67、p53表达的关系。 结果 肿块边缘模糊组的Ki-67阳性表达率(82.3%)高于边界清楚组(67.9%)及毛刺组(63.9%) (P< 0.05),肿块不规则组的p53阳性表达率(71.4%)高于类圆形组(65.5%)及分叶组(51.7%),无淋巴结转移组的p53表达阳性率(58.6%)高于有淋巴结转移组(40.9%)(P< 0.05)。 结论 乳腺X线征象肿块边缘模糊提示肿瘤组织中Ki-67可能高表达,肿块形态不规则、无淋巴结转移提示肿瘤组织中p53可能高表达。

     

    Abstract: Objective To explore the relationship between the mammographic appearances of breast infiltratingductal carcinoma and the expression of Ki-67 and p53. Methods The mammographic appearances of 189 patients with breast infiltrativeductal carcinoma confirmed by postoperative pathology in radiologydepartment of First Affiliated Hospital of Liaoning Medical University and The Fourth Hospital of Fushun Liaoning Province from August 2014 to September 2015 were retrospectively analyzed, and the relationships between mammographic appearances and postoperative pathological markers of Ki-67 and p53 were analyzed. Results The positive expression rate of Ki-67 in the obscured mass margin group was higher than circumscribed mass margin group and speculated mass margin group with statistically significantdifference (82.3% vs 67.9%, 63.9%, P< 0.05). The positive expression rate of p53 in irregular mass shape group was higher than oval mass shape group and lobular mass shape group (71.4% vs 65.5%, 51.7%), and it was higher in no lymph node metastasis group than lymph node metastasis group with statistically significantdifference (58.6% vs 40.9%, P< 0.05). Conclusion The mammographic appearances of obscured mass margin suggests high expression of Ki-67 in the tumor tissue, and irregular mass shape and no lymphatic metastasis suggests high expression of p53 in the tumor tissue.
