可手术乳腺癌患者的循环肿瘤细胞与临床病理特征的关系

Relationship between circulating tumor cells and clinicopathological features in patients with operable breast cancer

  • 摘要: 目的 探讨可手术乳腺癌患者外周血中循环肿瘤细胞(circulating tumor cells,CTCs)阳性率与其临床病理特征的相关性。 方法 2014年1 - 9月本院收治的73例可手术乳腺癌患者为研究对象,使用CellSearch系统检测所有患者在接受化疗前的CTCs,分析其与原发肿瘤临床病理特征和预后的关系。 结果 CTCs在可手术乳腺癌患者中的检出率为16.4%;CTCs阳性组与阴性组中肿瘤直径> 5 cm的患者分别占25%、3.3%,两组差异有统计学意义(P=0.036),且肿瘤直径是CTCs阳性的独立影响因素(OR=2.024,P< 0.05);CTCs阳性与阴性患者1年无病生存(disease free survival,DFS)率分别为80.8%、100%,两组差异有统计学意义(P=0.01)。 结论 原发肿瘤大小与可手术乳腺癌患者CTCs有关,但CTCs与其预后的关系尚不明确。

     

    Abstract: Objective To investigate the correlation between positive rate of circulating tumor cells (CTCs) and clinicopathologic features in patients with operable breast cancer. Methods From January to September in 2014, 73 operable breast cancer patients in Chinese PLA General Hospital were enrolled in this study. Their CTCs were analyzed before adjuvant chemotherapy using CellSearch system, and the relationship between CTCs and clinicalpathologic characteristics of primary tumor, prognosis was analyzed. Results The detection rate of CTCs in patients with operable breast cancer was 16.4%, and the detection rate of patients with tumor diameter greater than 5 cm in CTCs positive and negative group were 25% and 3.3% with statistically significant difference (P=0.036). Tumor size was independent risk factor for CTCs detection rate (OR=2.024,P< 0.05). The 1-year disease free survival (DFS) in CTCs positive and negative patients were 80.8% and 100% with statistically significant difference (P=0.01). Conclusion Tumor size is the only primary tumor characteristic that correlates with the presence of CTCs in patients with operable breast cancer. However, the relationship between CTCs and prognosis is not clear, long-term follow-up will be required to investigate the significance of CTCs in operable breast cancer.
