胃癌全胃切除术后早期肠内营养结合肠外营养的疗效观察

Efficacy of early enteral nutrition combined with parenteral nutrition for gastric cancer patients with total gastrectomy

摘要: 目的探讨早期肠内营养(early enteral nutrition，EEN)结合肠外营养(parenteral nutrition，PN)对胃癌全胃切除术后患者的疗效。方法回顾性分析2013年1月-2015年6月在本院行全胃切除术的胃癌患者108例，其中术后行早期肠内营养结合肠外营养(肠外营养即经静脉途径供应病人所需的营养)72例(EEN+PN组)，全肠外营养(total parenteral nutrition，TPN)36例(TPN组)。比较两组术前及术后7 d体质量、血清白蛋白、血红蛋白、术后排气排便时间、进流食时间、术后住院时间及各种并发症的发生情况。结果两组手术前后体质量、血清白蛋白、血红蛋白等营养状况指标及并发症发生情况差异均无统计学意义(P> 0.05)；与TPN组相比，EEN+PN组患者术后排气、排便、进流食及术后住院平均时间分别缩短了0.79 d、0.88 d、0.53 d和1.36 d(P< 0.05)。结论 EEN+PN在改善全胃切除术胃癌患者术后早期营养方面与TPN有同等效果，在促进胃肠功能恢复上效果更优，是一种安全、有效、合理的营养支持方法。
