伴发眩晕的突发性耳聋患者临床特征及诊治分析

Clinical features and treatment of sudden hearing loss in patients accompanied with vertigo

  • 摘要: 目的 分析伴发眩晕的突发性耳聋患者临床特征。 方法 回顾性研究我院2013年10月-2015年10月收治的60例(62耳)伴发眩晕的突发性耳聋患者的临床资料,包括性别、年龄、病程、眩晕持续时间、听力学特征、前庭功能及影像学特征等。 结果 60例患者,年龄15 ~ 65岁,平均40.5岁;男性32例(53.3%),女性28例(46.7%);单耳发病58例(左耳35例,右耳23例),双耳发病2例。听力损失程度以极重度为主(39耳,62.9%),听力曲线类型多为全聋型(30耳,48.4%)和下降型(18耳,29.0%);经内科常规治疗,本组患者听力恢复总有效率为46.8%,其中眩晕持续时间< 1 d者听力恢复较好(P< 0.05)。患侧前庭功能正常24例(40.0%),患侧前庭功能减退36例(60.0%),两组听力恢复程度无统计学差异(P> 0.05)。 结论 伴发眩晕的突发性耳聋临床发病率较高,听力损失程度较重,听力曲线类型以全聋型和下降型为主。前庭功能检查可表现为正常或减退,眩晕持续时间越长,临床治疗效果越差。

     

    Abstract: Objective To analyze the clinical characteristics of sudden sensorineural hearing loss (SSHL) patients with vertigo. Methods Clinical data including gender, age, course of disease, vertigo-duration, audiological examination, vestibular function examination and imaging examination were reviewed and analyzed in 60 SSHL patients (62 ears) with vertigo admitted to our hospital from October 2013 to October 2015. Results The average age of 60 patients was 40.5 years, ranging from 15 to 65 years old. There were 32 male patients (53.3%) and 28 female patients (46.7%). Most of them were affected monaurally with 35 patients in the left ear and 23 in the right ear, only 2 patients were affected bilaterally. Thirty-nine ears presented profound hearing loss, accounting for 62.9%. Besides, about 30 ears (48.4%) presented flat audiograms or total deafness, and 18 ears (29.0%) presented downslope audiograms. The total effective rate of these patients treated by conventional therapy were 46.8%, of them, patients with vertigo lasting less than 1 day presented better hearing recovery. There was no significant difference in hearing recovery between patients with vestibular dysfunction or not. Conclusion SSHL patients with vertigo is common seen in clinical setting with severe hearing loss and flat audiograms or total deafness in most of them. The vestibule function can be normal or abnormal, which is not correlated with hearing recovery. This study shows that the prognosis of SSHL patients with vertigo is associated with vertigo duration.
