大动脉粥样硬化合并脑白质病变的影响因素研究进展

Risk factors of coexisting white matter lesions in patients with large artery atherosclerosis

  • 摘要: 颅内大动脉粥样硬化(large artery atherosclerosis,LAA)与脑白质病变(white matter lesions,WML)累及血管不同,病理生理机制也不同,但二者累及血管在结构及功能上密不可分,存在诸多相同的血管危险因素,因此二者存在不容忽视的内在联系。LAA合并WML的影响因素尚不明确,可能包括:1) LAA会促进白质病变的发生及进展;2) LAA合并WML患者受血管危险因素的外在影响及遗传因素的内在作用。本文通过回顾LAA合并WML的临床及基础研究,探讨LAA合并WML可能存在的影响因素。

     

    Abstract: Large artery atherosclerosis (LAA) and white matter lesions (WML) involves different vessels with different pathophysiological mechanisms. However, the structurally and functionally connection of the different blood vessels and the common vascular risk factors of the two conditions suggest that they are inter-mingled and should be considered together. The possible risk factors of coexisting WML in patients with LAA include the occurrence of LAA contributing to WML progression, or more or severer vascular risk factors and genetic factors play important roles on the coexisting conditions. This article retrospectively reviews the basic and clinical researches in this field and summarizes the advances in risk factors of the coexisting conditions.
