Ki67、胎盘碱性磷酸酶在细胞角蛋白对颅内生殖细胞瘤和垂体瘤鉴别诊断中的价值

Value of Ki67, PLAP and CK in differential diagnosis of germinomas and pituitary adenomas

  • 摘要: 目的 探讨Ki67、胎盘碱性磷酸酶(placental alkaline phosphatase,PLAP)、细胞角蛋白(cytokeratin,CK)的表达在颅内生殖细胞瘤(鞍区)与垂体瘤鉴别诊断中的价值。 方法 选取我院36例颅内生殖细胞瘤患者和40例垂体瘤患者,免疫组化法测定两种肿瘤组织中Ki67、PLAP和CK蛋白的表达水平,分析两种组织中这3种蛋白表达的差异。 结果 Ki67、PLAP在颅内生殖细胞瘤中的阳性表达率分别为95.2%(20/21)和88.6%(31/35),显著高于在垂体瘤的阳性表达率47.6%(10/21)和16.7%(5/30)(χ2=11.667、33.800,P均< 0.05)。CK在颅内生殖细胞瘤的阳性表达率为27.8%(5/18),显著低于在垂体瘤的阳性表达率82.6%(19/23)(χ2=12.508,P< 0.001)。 结论 Ki67、PLAP和CK蛋白可作为辅助颅内生殖细胞瘤(鞍区)和垂体瘤鉴别诊断的生物学指标。

     

    Abstract: Objective To investigate the expressions of Ki67, placental alkaline phosphatase (PLAP), cytokeratin (CK)and their diagnostic values to germinomas and pituitary adenomas. Methods Thirty-six patients with germinomas (experiment group) and 40 patients with pituitary adenomas (control group) were selected. The expression of Ki67, PLAP and CK were detected by immunohistochemical assay, and the difference of their expression in two tumors was analyzed. Results The positive rates of Ki67,PLAP in germinomas were significantly higher than those in pituitary adenomas (Ki67:95.2% vs 47.6%, P=0.001; PLAP:88.6% vs 16.7%, P< 0.001). The positive rate of CK in germinomas was 27.8% (5/18), which was significantly lower than that in pituitary adenomas of 82.6% (19/23) (χ2=12.508, P< 0.001). Conclusion Ki67, PLAP and CK can serve as auxiliary biological indicators in differential diagnosis of germinomas and pituitary adenomas.
