肝前脂肪厚度对糖尿病患者代谢综合征的诊断价值

Value of hepatic fat thickness in diagnosis of diabetes complicated with metabolic syndrome

  • 摘要: 目的 探讨肝前脂肪厚度对糖尿病(diabetes mellitus,DM)患者代谢综合征(metabolic syndrome,MS)的诊断价值。 方法 收集2016年3月- 2018年3月在济南军区联勤部门诊部与济南军区总医院治疗的147例糖尿病患者的临床资料。根据所患疾病将其分为单纯糖尿病组与糖尿病并发代谢综合征组。比较两组的性别、年龄、总胆固醇、低密度脂蛋白、腰围、体质量指数(body mass index,BMI)、肝前脂肪厚度,并绘制肝前脂肪厚度与腰围单独或联合诊断MS的ROC曲线,评估诊断价值。 结果 DM & MS组总胆固醇、腰围、BMI、有家族史患者比例、肝前脂肪厚度均显著高于单纯DM组(P均< 0.05)。男性患者肝前脂肪厚度、腰围及二者联合诊断MS在ROC曲线下面积分别为0.856、0.755、0.920,女性则为0.868、0.754、0.917。 结论 肝前脂肪厚度对糖尿病患者代谢综合征诊断具有参考价值。

     

    Abstract: Objective To explore the diagnostic value of prehepatic fat thickness in patients with diabetes complicated with metabolic syndrome. Methods Clinical data about 147 diabetic patients who were treated in outpatient department of joint service in Ji'nan Military Region and General Hospital of Ji'nan Military Area from March 2016 to March 2018 were collected. Patients were divided into simple diabetes (DM) group and diabetes complicated with metabolic syndrome (DM & MS) group. Data including gender, age, TC, LDL, waist circumference, BMI and prehepatic fat thickness were compared between the two groups. The prediction value of prehepatic fat thickness, waist circumference and their combination for MS was evaluated by ROC analysis. Results The total cholesterol, waist circumference, BMI, proportion of patients with family history, and hepatic fat thickness in the DM & MS group were significantly higher than those in the simple DM group (all P< 0.05). In male patients, the area under the ROC curve was 0.856, 0.755, and 0.920 for the hepatic fat thickness, waist circumference, and their combination, and they were 0.868, 0.754, and 0.917 for female. Conclusion Prehepatic fat thickness can be used as an indicator for the diagnosis of diabetic metabolic syndrome.
