扁桃体癌患者人乳头瘤状病毒DNA整合位点分布分析

Analysis of distribution of human papillomavirus DNA integration sites in tonsil carcinoma

  • 摘要:
      背景  扁桃体癌是我国常见的头颈部恶性肿瘤之一,近年来人乳头状瘤病毒(human papilloma virus,HPV)相关扁桃体癌已被划分为一类独立的疾病。
      目的  通过HPV捕获和二代测序技术分析扁桃体癌患者HPV的感染情况以及HPV DNA整合到人类基因组中的位点,揭示HPV相关扁桃体癌的致病机制。
      方法  收集2015年10月 − 2020年7月解放军总医院耳鼻咽喉头颈外科收治的扁桃体鳞癌患者的肿瘤组织新鲜冷冻样本或石蜡包埋样本,使用CTAB法提取肿瘤组织样本DNA,运用Covaris打断法、PCR扩增及产物纯化制备DNA全基因组文库,使用根据17种HPV型(6、11、16、18、31、33、35、39、45、52、56、58、59、66、68、69、82)的全基因组设计的HPV探针捕获目标基因,使用Illumina HiSeq 2000测序仪进行测序,并进行生物信息学分析。
      结果  32例扁桃体癌组织样本主要检测出5种HPV类型(HPV16、18、33、53、67);26例样本HPV DNA呈阳性;22例HPV16 DNA阳性的样本中,有20例出现HPV整合。此外,发现了MIPOL1和CDK12两个致癌热点基因。
      结论  本研究扁桃体癌患者HPV阳性率与国内外其他研究整体上一致,MIPOL1和CDK12两个热点基因可能与HPV阳性扁桃体癌的发生相关。

     

    Abstract:
      Background  Tonsil carcinoma is one of the common malignant tumors of the head and neck in China. In recent years, HPV-related tonsil carcinoma has been classified as an independent disease.
      Objective  To analyze the HPV infection of tonsil squamous cell carcinoma patients and the site of HPV DNA integration into the human genome by HPV capture and next generation sequencing.
      Methods  Tumor tissue samples (fresh frozen samples or paraffin-embedded tissue samples) of tonsil squamous cell carcinoma patients who admitted to Chinese PLA General Hospital from October 2015 to July 2020 were collected. CTAB method was used to extract DNA from tumor tissue samples, and Covaris interruption method, PCR amplification and product purification were applied to make a DNA whole genome library. The target genes were captured by HPV probe, which was designed according to whole genome of 17 HPV types (6, 11, 16, 18, 31, 33, 35, 39, 45, 52, 56, 58, 59, 66, 68, 69, 82). Then the Illumina HiSeq 2000 was used for sequencing, and finally bioinformatics analysis was conducted.
      Results  Five major HPV types (HPV16, 18, 33, 53 and 67) were detected in 32 samples; and 81.3% (26/32) of the samples were positive for HPV DNA; HPV integration occurred in 90.9% of HPV16 DNA positive samples. Furthermore, MIPOL1 and CDK12 were also discovered.
      Conclusion  In this study, the HPV positive rate in tonsil carcinoma patients is generally high. It is found that the two hot spots of MIPOL1 and CDK12 may be related to the occurrence of HPV-positive tonsil carcinoma.
