过敏性鼻炎/哮喘患者血清过敏原及特异性IgE水平分析

Analysis of distributions of inhaled allergen and specific IgE levels in patients with allergic rhinitis/asthma

      背景  过敏性鼻炎和(或)哮喘是最常见的过敏性疾病类型,而引起上述疾病的各种吸入过敏原的阳性率、特异性IgE(specific IgE,sIgE)平均水平、不同水平sIgE的分布特征以及sIgE与相关指标的关系并不明确。
      目的  观察不同过敏原sIgE的水平及其与相关指标的关系。
      方法  选择2016年10月- 2020年1月因过敏性鼻炎和(或)过敏性哮喘到我院变态反应门诊就诊的1 101例患者,观察10种吸入过敏原血清sIgE的阳性率、sIgE平均水平、不同级别sIgE的分布特征以及sIgE与年龄的相关性。
      结果  纳入病例中男性378例,平均年龄(37.42 ± 16.78)岁,女性723例,平均年龄(38.79 ± 14.62)岁。过敏原中秋季花粉的阳性率最高(35.79%),sIgE的水平也明显高于其余吸入过敏原(P < 0.05),且在特重度过敏患者中比例最高(45.18%)。尘螨阳性率34.51%,是最常见的常年过敏原,sIgE水平与霉菌分布相似。狗毛、霉菌混合sIgE水平与年龄呈正相关(r=0.231,P=0.005;r=0.216,P=0.000),苋花粉、秋季花粉sIgE水平与年龄呈负相关(r=-0.147,-0.146)(4项P < 0.05)。
      结论  秋季花粉和尘螨分别是最常见的季节和常年过敏原,过敏患者血清中不同过敏原sIgE的水平有很大差异,但多数不受性别和年龄影响。

     

    Abstract:
      Background  Allergic rhinitis and/or asthma is one of the most common types of allergic diseases. However, the positive rate of various inhaled allergens, the mean level of specific IgE (sIgE), the distribution characteristics of different levels of sIgE, and the relationship between sIgE and related indicators are not clear.
      Objective  To observe the levels of different allergen sIgE and their relationships with related indexes.
      Methods  A total of 1 101 patients with allergic rhinitis and/or allergic asthma admitted to the allergic reaction clinic of our hospital from October 2016 to January 2020 were selected. The positive rate of serum sIgE, average level of sIgE, distribution characteristics of different levels of sIgE and the correlation between sIgE and related indicators were observed.
      Results  There were 378 males with an average age of (37.42 ± 16.78) years and 723 females with an average age of (38.79 ± 14.62) years. The positive rate of autumn pollen allergy patients was the highest (35.79%). The level of sIgE was significantly higher than other inhaled allergens (P < 0.05), and the proportion of extremely severe allergy patients was the highest (45.18%). The positive rate of dust mite sIgE was 34.51%, which was the most common perennial allergen. The level of sIgE distribution of dust mite was similar to mold. The sIgE levels of dog dander and mixed mold were both positively correlated with age (r=0.231, P=0.005; r=0.216, P=0.000), while the sIgE levels of amaranth pollen and autumn pollen were negatively correlated with age (r=-0.147, -0.146; all P < 0.05).
      Conclusion  Autumn pollen and dust mite are the most common seasonal and perennial allergens, respectively. The levels of different allergen sIgE in patients have great differences, but most of them are not affected by gender and age.
