某部队近5年军事训练伤调查分析

A retrospective analysis in military training injuries in recent 5 years in a group army

      背景  军事训练伤是影响官兵健康、削弱部队战斗力的重要因素,目前尚缺乏对特定体系部队军事训练伤发生现状的大样本调查研究。
      目的  分析某部队近5年军事训练伤的发生特点及规律,为优化军事训练伤防护对策提供依据。
      方法  利用某军队卫勤统计系统收集2015 - 2019年某部队符合军事训练伤的官兵数据资料,从伤员基本情况、伤残等级分布、训练伤病种分类、训练伤部位分类和训练伤月度分布等方面进行综合分析判断。
      结果  2015 - 2019年共发生军事训练伤3 712例,21 ~ 30岁年龄组占比最高(60.75%),军士是最大发病群体(51.56%)。伤残等级集中在7 ~ 10级(98.38%)。骨关节损伤2 628例(70.80%),软组织损伤944例(25.43%),器官损伤140例(3.77%)。骨关节损伤中前3位是骨折(43.38%)、半月板损伤(21.69%)和腰椎间盘突出(13.51%)。软组织损伤中韧带损伤(79.77%)最常见,其次为肌腱损伤(14.41%)。器官损伤中耳损伤(42.14%)和眼损伤(19.29%)居多。下肢损伤比例最高(55.39%),其次是上肢(24.62%)和躯干部(16.38%)。下肢损伤部位中前3位是膝关节(61.53%)、踝关节(19.21%)和股骨(6.03%)。上肢损伤最常见于肩关节(38.18%),躯干损伤最常见于腰椎(90.13%)。每年3 - 5月和10 - 12月是训练伤发生的高峰时段,分别占总数的27.75%和29.58%。
      结论  该体系部队军事训练伤好发于年轻军士,以骨关节损伤为主,骨折是其主要表现形式。损伤部位多为下肢,尤以膝关节最多。3 - 5月和10 - 12月是军事训练伤的高发期。

     

    Abstract:
      Background  Military training injury (MTI) is an important factor affecting the health of officers and soldiers, and weakening the combat effectiveness of troops. However, there is still a lack of large sample investigation and research on the occurrence of MTI in a certain system of troops.
      Objective  To analyze the characteristics and regularity of MTI in a certain system of troops in recent 5 years, and provide reference for optimizing MTI protection countermeasures.
      Methods  Based on the MTI data from 2015 to 2019 collected by a certain military medical service statistical system, a comprehensive evaluation was made from the aspects of basic information of the personnel, distribution of disability levels, classification of injury types and injured parts, and monthly distribution of MTI.
      Results   A total of 3 712 cases of MTI occurred from 2015 to 2019. The largest percentage of MTI (60.75%), occurred in the 21-30 years old group, and the military sergeants were involved most frequently (51.56%). During the 5 years, there were 2 628 cases had bone and joint injury (70.80%), 944 cases had soft tissue injury (25.43%), and 140 cases had organ injury (3.77%). Fracture accounted for the largest proportion(43.38%) of the bone and joint injuries, followed by meniscus injury (21.69%) and lumbar disc herniation (13.51%). Ligament injury was the most common type of soft tissue injuries(79.77%), followed by tendon injury (14.41%). The most common organ injuries were ear injury (42.14%) and eye injury (19.29%). In terms of injury site, the proportion of lower limb injury was highest (55.39%), followed by upper limb injury (24.62%) and trunk injury (16.38%). The top 3 injury sites of lower extremity were knee joint (61.53%), ankle joint (19.21%) and femur (6.03%). Shoulder joint (38.18%) was the most common in the upper limb injuries, while lumbar spine (90.13%) were the most common in the trunk injuries. March-May and October-December were the peak periods of MTI, accounting for 27.75% and 29.58%, respectively.
      Conclusion  MTI are common among young male sergeants, mainly including bone and joint injuries, and fracture is the main manifestation. Most of the injuries involve the lower limbs, especially the knee joint. March-May and October-December are the periods of high incidence of MTI.
